TB-500

A healing peptide for muscle strains, larger soft-tissue injuries and the stiffness that follows them.

TB-500 is a synthetic version of thymosin beta-4, a protein found in nearly every cell in your body and concentrated in wound fluid and healing tissue. Its job is to organize actin, the scaffolding cells use to move. In practical terms, that means it helps repair cells travel to an injury, helps new blood vessels form, and keeps healing tissue flexible rather than stiff and scarred.

Dr. Loera uses TB-500 for muscle strains and tears, larger soft-tissue injuries, and the stiffness and reduced range of motion that linger after them. It is frequently paired with BPC-157, since the two cover different parts of the healing process. See our injury recovery page for how they fit into a full plan.

How it helps

What TB-500 does

Where BPC-157 is the tendon specialist, TB-500 is the mobility and soft-tissue specialist.

Cell migration

TB-500 helps repair cells move into damaged tissue, one of the first steps in healing.

New blood vessels

It promotes angiogenesis, supplying injured muscle and connective tissue with oxygen and nutrients.

Muscle repair

Strains, tears and post-workout damage recover faster with less scar formation.

Reduced inflammation

It dampens inflammatory signaling, easing pain and swelling during recovery.

Flexibility and range of motion

Healing tissue stays more pliable, which is why TB-500 is popular for stiff shoulders, hips and backs after injury.

Wound and skin healing

Thymosin beta-4 is also studied for wound closure and hair follicle support.

What we use TB-500 for

  • Muscle strains and tears: hamstring, calf, quad, groin, pectoral
  • Larger soft-tissue injuries and deep bruising
  • Stiffness and limited range of motion after injury or surgery
  • Chronic back, hip and shoulder tightness with an injury history
  • Tendon and ligament injuries, usually alongside BPC-157
  • Slow-healing wounds

How TB-500 is dosed

TB-500 has a longer action than most peptides, so it’s typically injected two or three times a week rather than daily. Protocols usually begin with a four-to-six-week loading phase at a higher weekly dose, then drop to a maintenance dose every week or two if the injury is chronic. Because it works systemically, the injection site doesn’t need to be near the injury; most people use the abdomen.

TB-500 vs. BPC-157

They’re often confused because they’re often prescribed together. BPC-157 is strongest for tendons, ligaments, the gut lining and localized healing. TB-500 is strongest for muscle, large soft-tissue injuries and keeping healing tissue flexible. For a single tendon problem, BPC-157 alone usually does the job. For a muscle tear, significant injury or a chronic problem with stiffness, the pair is the common choice.

Candidates

Is TB-500 right for me?

Often a good fit

  • A muscle strain or tear that keeps nagging or re-tearing
  • A larger soft-tissue injury or deep bruise that’s slow to resolve
  • Stiffness or lost range of motion after an injury or surgery
  • An active adult or athlete wanting to shorten recovery
  • Comfortable with a few self-injections per week

Let’s talk first

  • Active cancer or cancer treatment in the last five years (TB-500 promotes blood-vessel growth)
  • Pregnancy or breastfeeding
  • A complete muscle or tendon rupture needing surgical evaluation first
  • Competing athletes subject to anti-doping rules (TB-500 is prohibited)
  • A bleeding disorder or blood thinners (we’ll review)

The process

How it works

  1. Step 1

    Consultation

Dr. Loera examines the injury, reviews your history and imaging, and decides whether TB-500 alone or with BPC-157 fits best.

  2. Step 2

    Your protocol

Loading and maintenance doses, injection schedule and a mobility or rehab plan to run alongside.

  3. Step 3

    First dose and teaching

We show you the injection technique in the office so the at-home doses are easy.

  4. Step 4

    Four to six weeks

Prescriptions ship from a licensed compounding pharmacy. Two to three injections a week during the loading phase.

  5. Step 5

    Check-ins

Follow-up at two to three weeks and at the end of the loading phase to decide on maintenance or stopping.

Good to know

TB-500 questions, answered

What is TB-500?

TB-500 is a synthetic fragment of thymosin beta-4, a naturally occurring protein that helps cells move, form new blood vessels and repair damaged tissue. It’s used in regenerative medicine for muscle, soft-tissue and connective-tissue injuries.

What is TB-500 best for?

Muscle strains and tears, larger soft-tissue injuries, and stiffness or lost range of motion after injury. It is also used, usually with BPC-157, for tendon and ligament problems and for slow-healing wounds.

How long does TB-500 take to work?

Many people notice reduced pain and better mobility within two to four weeks of starting the loading phase. Full tissue repair continues over the course and for weeks after.

How often do I inject TB-500?

Usually two or three times a week during a four-to-six-week loading phase, then once every week or two for maintenance if the injury is chronic. It works throughout the body, so injection site isn’t critical.

Is TB-500 FDA approved?

No. TB-500 is not an FDA-approved drug. It is prescribed off-label and prepared by compounding pharmacies, and availability can change as regulations evolve. Dr. Loera will confirm what can currently be prescribed.

What are the side effects of TB-500?

It is generally well tolerated. Some people report brief fatigue, a mild headache or lightheadedness after the first few doses, plus injection-site redness. Because it promotes blood-vessel growth, we avoid it in people with active cancer.

Should I take TB-500 with BPC-157?

For a significant injury, a muscle tear or a chronic problem with stiffness, the combination is common and the two complement each other well. For a single tendon issue or gut problem, BPC-157 alone is usually sufficient. We’ll recommend based on your injury.

Is TB-500 banned in sports?

Yes. Thymosin beta-4 and its fragments are on the WADA prohibited list. If you compete in a tested sport, tell us before starting.

Can TB-500 help with hair growth?

Thymosin beta-4 has been studied for hair follicle activity and wound healing, but that is not a primary use in our practice. If hair is your goal, GHK-Cu or exosome treatments are better-supported options we can discuss.

How much does TB-500 cost?

It depends on the dose and course length. Peptide therapy is self-pay and not covered by insurance. You’ll get a clear quote at your consultation, and our price list is always available.
